Biofourmis

Biofourmis is a US-based healthcare technology company that provides technology-enabled programs for delivering care at home. Its connected platform supports Hospital at Home, Timely Discharge, SNF at Home and Remote Patient Management services, allowing patients to receive acute and post-acute care at home and move between levels of care. Biofourmis works with more than 50 health systems and payers and has licensed operations in 25 US states. In October 2024 it merged with CopilotIQ, and in January 2025 it sold its life-science business to ActiGraph, focusing on care delivery.
